📌 Who Needs a Physical?

  • All incoming 7th, 9th, and 11th graders
  • Any student who did not get a physical last year
  • Any student who answers “Yes” to questions 1–6 on the Medical History Form

📌 Who Needs to Fill Out the Medical History Form?
✅ EVERYONE. This form must be completed every year, regardless of whether a physical is required.
